Directory Form (re-posted 7/5)
If you have already filled out a new form in the past couple of months--that's all you need to do. For those who haven't, please use one of the versions available on the website to complete. There are two different versions posted online under Communications--one is a form that you print out and fill in by hand, sign and return. The other form can be filled out directly from your computer (and then we don't have to worry about handwriting--do we?), printed, signed and then turned in at Membership or Activities. Both forms can be reached by clicking here. Thank you for your help and cooperation in this project.
